Select how severe a health and safety incident was, using a clear visual scale of options.

Steps​

Step 1: Open the incident severity step​

Navigate to the "How bad was it?" step in the incident workflow to view the severity question.

Step 2: Review the full severity scale​

View all available severity options displayed as individual cards, from the most serious incident at the top to the least serious at the bottom.

Step 3: Select the appropriate severity​

Click the card that best matches how serious the incident was, such as "Fatality", "Major injury", or "No injury".

Step 4: Confirm your selection and continue​

Check that the correct severity card is highlighted, then continue to the next step of the incident workflow.
