Issue a project form
Use this dialog to issue a form for your project, choosing the template, version, recipient type, and access options before sending or sharing the form.
This guide is designed for project manager.
Steps
Step 1: Open the issue form dialog
From your project workspace, open the issue form dialog (for example via an “Issue form” or “New issue” button); the dialog appears with options to configure a new form instance.

Step 2: Select a form template
In the Template section, open the template dropdown, search or scroll through the available templates for this project, and select the template you want to issue; the dialog loads the related versions and shows a template preview.

Step 3: Choose the form version
Use the Version dropdown to select “Latest” or a specific version of the template so you can control exactly which set of questions is issued.

Step 4: Pick how the form will be issued
In the Issue type area, choose whether the form is Assigned (one named recipient), Open Link (anyone with the link), or Anonymous (recipients are not identified); review the description beneath each option to confirm it suits your use case.

Step 5: Configure recipient and access options
Depending on the issue type, select whether recipients are internal or external, and adjust any visibility, privacy, or access options so only the right people can complete the form.

Step 6: Review the template preview
Scroll through the Template preview section to confirm the questions and layout match what you expect before you issue the form.

Step 7: Issue the form and get sharing options
Click the button to issue the form; once created, you can copy the public link, download a QR code, and, if applicable, send it directly to the chosen recipient.
